Products - Long Term Disability
Monthly Benefit
will be paid after the Elimination Period when we receive
proof that the insured is disabled and is under the regular
care of a Physician. The benefit will be paid up to the Maximum
Benefit period, as long as the insured remains disabled and
under a Doctors care.

Employees may pay some, all, or none of the premium.

Age
Elimination period
means the number of days that the insured must be disabled
before benefits begin.
Three Month Survivor
Benefit pays a lump sum benefit equal to three times
the insureds last gross monthly benefit payment, to
an eligible survivor, if an employee dies after being disabled
for 6 months or longer and while receiving benefits.
Mental Illness
Limitation provides coverage for up to one or two years.
The limitation can also include Self Reported Symptoms. This
limitation is optional for groups with 50 or more lives and
if included, is a cost containment feature.
Temporary Recovery
- If a disabled employee who was receiving benefits returns
to work for less than six months and has a recurrence of the
same disability, that disability will be considered a continuation
of the old one, and a new Elimination Period will not have
to begin.
Definitions
Of Disability
Limited (12, 24, 36, or 60) Months
Regular Occupation
* Must be totally disabled from regular occupation
through out the Elimination Period to qualify for benefits.
* After the limited number of months, must be totally disabled
from any gainful occupation.
* If Residual is included, partial disability may be used
to satisfy the Elimination Period as long as there is a 20%
or more loss of earnings.
Extended Regular
Occupation
* Must be totally disabled from regular occupation
through out the Elimination Period to qualify for benefits.
* There is no cut off period as the regular occupation
period is unlimited.
* If Residual is included, partial disability may be used
to satisfy the Elimination Period as long as there is a 20%
or more loss of earnings.
Limited 24 Month/Activities
of Daily Living
* Must be totally disabled from regular occupation
through out the Elimination Period to qualify for benefits.
* Benefits will continue beyond 24 months if the insured remains
disabled and is unable to perform one or more Activities of
Daily Living, without standby help, or has a cognitive impairment
or a terminal illness.
* If Residual is included, partial disability may be used
to satisfy the Elimination Period as long as there is a 20%
or more loss of earnings.
General Exclusions
- Disabilities resulting from war, active participation in
a riot, intentionally self inflicted injuries, or attempt
to commit a felony are not covered.
Waiver Of Premium
- When an employee is receiving LTD benefits, any premium
that becomes due is waived.
Pre-existing
Condition - Disabilities due to a preexisting condition
are not covered for certain periods of time. A pre-existing
condition is a sickness or injury the insured received treatment
for or where symptoms were present within a certain period
of time prior to the insureds effective date of coverage.
The preexisting condition limitation included is determined
by the size of the group.
Social Security
Integration - Primary and Family, or Primary only.
All Sources
- A 60% or 70% All Sources Benefit is also available for some
groups. This means that a disabled employee will not receive
less than 60 or 70% of pre-disability earnings, including
Other Income Benefits.
Cost Of Living
Increases - After the first deduction for each of the
Other Income Benefits, the monthly benefit will not be further
reduced due to any cost of living increases payable under
these Other Income Benefits.
Other Income
Amounts To prevent overinsurance, LTD payments will
be reduced by income the insured receives or is eligible to
receive from Social Security, workers' compensation, group
disability plans with employer or association, employer retirement
plans, formal salary continuation or sick leave plans, state
disability plans, unemployment benefits, and etc.
Minimum Monthly Benefit
- A disabled employee will never receive less than the greater
of 10% of the benefit before any reductions for Other Income
Benefits, or $100.00. A $50.00 minimum monthly benefit will
be included for some groups.
Definition Of
Basic Monthly Earnings means an employees basic
rate of pay. Overtime pay, bonuses, and extra compensation
are generally excluded. Commissions may be included. Other
definitions are available.
Additional
Benefits included without additional premium:
Workplace Modification Benefit
reimburses the employer for modifications to the workplace
to accommodate a disabled employee.
Vocational Rehabilitation
Service assists the disabled employee in returning
to full or part-time employment.
Social Security
Assistance is available to the disabled employee in
applying for Social Security benefits and assists the employee
through the appeal process.
Optional Benefits:
Six Month Survivor Benefit in
lieu of the Three Month Benefit.
Escalation Benefit
increases the disabled employees monthly benefit by
1 % or the change in the CPI index, for up to 5 adjustments.
Retirement Income
Protection pays the disabled employee an additional
benefit to continue contributions to a 401 (k) Plan or Employer
Pension Plan.
Spousal Disability
Benefit provides a benefit to an employees disabled
spouse for up to 24 months. The spouse must be unable to perform
two or more Activities of Daily Living without standby help,
has a cognitive impairment or a terminal illness.
