How to Get a Group Proposal
from United Heritage Life Insurance Company
To prepare a group proposal we need:
- Age or Date of Birth
- Gender
- Salary (for disability or salary
plans only)
- Job Title
- 3 years experience for:
- STD if 100 or more lives
- LTD if 300 or more lives
- Life if 1,000 or more lives
- Any other information you can provide
is helpful (copy of current plan, rates, etc.)
