Amounts of Insurance - Plan design is flexible depending on the needs of the employer and the employees.

Benefit Option
Day Benefits Begin:
Accident-
1st 4th 8th 15th 22nd 31st day
 
Sickness-
  4th 8th 15th 22nd 31st day

% of Salary Replaced - 40% up to 70%
Maximum Weekly Benefit - $100 up to $1,000 (higher benefit may be available)
Maximum Benefit Period - 9 weeks up to 52 weeks  
Employees may pay some, all, or none of the premium.

Definitions of Disability
- A non-occupational sickness or injury that prevents the insured from doing any of the material and substantial duties of the insured's regular occupation.
- A non-occupational sickness or injury that prevents the insured from doing some or all of the material and substantial duties and has at least a 20% loss in earnings. Residual may be included which allows the insured to work part-time or reduced hours to count towards satisfying the Elimination Period.

Working Benefit - If an insured is disabled and working, earning between 20% and 80% of pre-disability earnings, the insured will still be eligible for a benefit.

Minimum Payment Amount - The minimum benefit is $25 per week.

Temporary Recovery - If a disabled employee who was receiving benefits returns to work for less than 14 days and has a recurrence of the same disability, that disability will be considered a continuation of the old one, and a new Elimination Period will not to have begun.

Other Income Amounts - STD coverage is designed to replace a reasonable portion of a disabled insured's salary. To prevent overinsurance, STD payments will be reduced by income the insured receives or is eligible to receive from Social Security. Employer retirement plans, State disability plans, Unemployment benefit, etc. STD payments are NOT reduced by 401(k) plans, thrift plans, profit sharing plans, individual disability plans, etc.

General Exclusions - Disablilities resulting from war, active participation in a riot, intentionally self inflicted injuries, attempt to commit a felony, or an occupational sickness or injury, are not covered.
